This multi-event consent form is used by Jesus Youth UK Teens Ministry to obtain parental consent for children to attend ongoing events or multiple events organised by the ministry. All children under 18 must have parental consent before they can attend a Jesus Youth event. Your child will not be admitted to the event without a completed consent form. The purpose of this form is to ensure that parents or guardians are fully informed about the details of the event, for organisers to be made aware of any potential risks, and to obtain parents’ consent for their child's participation. This form will remain valid for the duration of the child’s involvement in the named event and will only need to be completed once. However, if there are any changes to the information provided, parents will be required to review and re-sign the form.
